To make an appointment with Dr Ben Stephens, please provide a referral from your General Practitioner or Specialist via email or fax.

To make an appointment with Dr Ben Stephens we require a referral from your General Practitioner or Specialist in the first instance.
Referrals can be sent to us via Healthlink, emailed to office@drbenstephens.com.au or faxed and must be in scanned PDF format (not an image or converted image to PDF from a phone).
When we receive a referral, Dr Stephens will review and triage the appointment. Then, we will give you a call to schedule an appointment.
If you want to claim a Medicare rebate, you must have a valid referral. Please note the following information regarding valid referrals:

If you want to claim a Medicare rebate, you must have a valid referral. Please note the following information regarding valid referrals:
- General Practitioner and Specialist referrals are valid for 2 years
- Once the referral has been started, the validity for General Practitioner referral is 12 months – the validity for a Specialist referral is just 3 months
- A new referral is needed for each new issue
- A referral doesn’t necessarily need to be made out to Dr Ben Stephens, as long as the referral is intended for a doctor of the same specialty (eg. Obstetrician & Gynaecologist).
No Valid Referral
Without a valid referral you will not qualify for a Medicare rebate and will have to pay the full fee out of pocket.
